The hotbar will sometimes show as scrolled to a slot without the player holding the item in that slot in their hand. The item in the hotbar slot cannot be used until the player scrolls away from that slot then back. It happens controllers, scroll wheels, and number keys. This is a VERY important bug as it kills many people in pvp since they cannot swap to the items in their hotbar quickly or reliably at times.
