In a survival world, I had a breakout in my mob spawner. I ended up dying, and I ran back down with a nearly broken iron sword to find my gear completely gone. Multiple hours of work gone to waste because it all despawned immediately after death. It disappeared in the death screen, and I assumed it was in the wall, but tearing apart the wall proved otherwise.
