I will occasionally respawn at the wrong y level after dying, on both single player and multiplayer worlds, using a bed as my spawn point.

An example of this was when me and my friend were playing. My friend died, respawn at the wrong y level, in a dungeon, was bombarded by zombies, and died multiple times, always re spawning in the same dungeon.
