OK, so I've heard those stories, but I had a different cause. It was because of long-pressing paintings. I accidentally did it first on 0.12.1, but it also did on 0.13.0 so yah, I think it applies to all versions. It only applies to the affected world; and on servers. One time I was on servers; someone killed me with a bowl!!! It was annoying!! I was unable to hit him with another object but weapons. This may not be a duplicate since you heard my story.
