The issue I experienced was that I placed all my gear, weapons and armour, into item frames on the wall next to me (all within 4 or 5 blocks of me) before going afk farming. My system then crashed to desktop and when I reloaded 3 of the item frames were empty and I was missing my diamond helmet, trousers and sword.
