When switching from Creative to Survival and then activating a Nether Reactor,and after I picked up the items,and the Reactor turns off,I switched back to title and went to the same world(I maybe did that).Then, when I looked at the Hotbar,some icons were changed.It doesn't affect the held item in Steve's hand and the icon from my inventory.
